To schedule an appointment, please call our office at (541) 926-6911 or request an appointment online. We accept cash, check, and major credit cards, and we will submit your insurance forms for you. Cancellation Policy
We ask that you kindly provide us with at least 24 hours' notice if you are unable to keep an appointment. A fee may be charged if a patient does not show up for an appointment without sufficient notice.
